
INSURANCE AND FEES:
Our practice is committed to providing you with the best possible care. We will also help you receive your maximum allowable benefits, however in order to do this, we need your assistance in understanding our financial policy. Dental insurance is a contract between you and your chosen insurance company. Understanding your plan's covered benefits, percent of coverage, deductibles and yearly maximum will help you fully utilize the benefits allowed under your plan, and manage your financial obligations.
Our fees are based upon a combination of our costs, our time and constant dedication to supplying our patients with the highest quality of dental care. The treatment recommended by us is never based on what your insurance company will pay, but on clinical findings. We believe your treatment should not be governed by insurance companies and therefore we do not enter into contracts with them. While filing of all insurance claims is a courtesy we extend to our patients, all charges are ultimately the patient's responsibility.
For restorative work, we require co-payment to be paid at time of service. Amount of co-pay is based on a percentage of total amount due. Any remaining balance after your insurance pays remains you responsibility. Your prompt remittance is appreciated.
Most insurance companies will pay us directly for your submitted claim, however there are several companies that send payments to you, the subscriber. In these instances, payment is expected in full on the date of service.
